  2. 2Notify healthcare providersIf your loved one was receiving care, let their providers know so they can assist with arrangements.
  3. 3Contact the relevant authoritiesDepending on the circumstances, emergency services or the police may need to be notified.
  4. 4Gather important documentsIdentification (needed for the death certificate), insurance policies and legal papers.
  5. 5Request medical recordsApply at the hospital; an application fee may apply and processing usually takes 2–3 weeks.
